D224 Global and Population Health Activity Log More information about D224 Global and Population Health Activity Log

The purpose of this field experience is to expose you to nursing care interventions that focus on a population, which may differ from past courses in which you may have worked with a single or limited number of patients. In this field experience, you will engage with a community health nursing professional (BSN required) employed at a community facility working with populations requiring preventative and restorative/regenerative care. You will be required to complete your field experience hours observing this community health nursing professional (BSN required) providing preventative care (i.e., prevention of disease, injury, disability, or premature death before they occur) and restorative/regenerative care (i.e., restoring back to health following recovery from illness, disease, exacerbation of a chronic illness, or trauma).

“Of the total 35 direct precepted hours, you are required to complete at least 10 hours in preventative health or at least 10 hours in restorative/regenerative health, with the remaining 25 hours in either area.”

  • Complete date and time to reflect the hours spent with your preceptor. You may make 2 entries if you have observed both preventative and restorative resources during your shadow day. Divide the hours as needed by what you have observed. For example if you have shadowed an 8 hour day and approximately 3 hours have been preventative and 5 hours have been restorative you may make one 3 hour entry and one 5 hour entry for the same day.  Be mindful of the times you add to your submission–don’t ‘double dip’ hours (example and 8 hour day 7am-3pm you would separate hours 7am-10am for preventative and 10am-3pm for restorative).
  • Make sure ALL hours are rounded up to half/whole hours—examples: 2hr45min=3hr, 1hr7min=1.5hr, 4hr40min=5hr
  • If you choose Restorative then describe ‘exactly’ the resources and interventions you observed the RN using that were restorative (secondary).  Be specific on what the resource is that you observed the preceptor using with patients.
  • In the description area describe how this restorative resource helped to move this patient either back to base-line or increased with wellness state.
  • If you choose Prevention then describe ‘exactly’ the resources and interventions you observed the RN using that were preventing a problem (primary).  If education was given don’t just state ‘education given’ but instead state ‘exactly’ what the education was.
  • In the description area state what the preventative resource/intervention is preventing.  For it to be a preventative resource this will need to actually prevent a problem before it has become a problem.  Remember treatment and screening is NOT prevention (primary).  It is secondary health. An example of a prevention resource/intervention might be: ‘patient was educated on how to use a walker with a return demonstration to ensure patient understanding.—then in the description area; ‘education on how to use a walker will help to prevent a fall from occurring.’
