******I work in a psychiatric facility, my quistion would be “do the patient who utilize aftercare resources set up at the time of discharge have lower rehospitalization rates. How often are patient who do not use discharge community resources more likely to readmit with in 30 days.”
– I work with alot of homeless at the city anchor hospital, these patients often less likely to follow up due to substance use, lack of transportation or mental health problems. IE. a homeless schizophrenic patient who does not follow up or take medicine they are discharged with is more likely to need readmission. ******
Assessment Deliverable
Write a 700- to 1,050-word paper summarizing your selected problem and associated intervention plan that includes a title page and references page and addresses the following:
- Using your current clinical setting, decide what QI problem you would like to tackle (e.g., patient satisfaction, fallrates, surgical site infections, ventilator acquired pneumonia).
- Provide a clinical question you would like to answer (e.g., “In adult ICU patients who are mechanically ventilated, does using a prone positioning protocol improve oxygenation and decrease time on the ventilator over a 30-day period?” OR “With homecare patients who are receiving multiple medications, does the use of pill packets for prescribed medications improve patient medication adherence compared to a multiple-medication pill tray?”).
- Identify an evidence-based intervention you could reasonably implement and evaluate (e.g., fall risk signage, wound care dressing protocol, prone position protocol). Briefly, describe the intervention.
- Describe the data you will collect. Include a description of the level of measurement of your data (i.e., nominal, ordinal, interval, ratio).
- Discuss over what period you will collect your data and why (e.g., 14 days, 30 days, 60 days).
- Explain how you might present a summary of your data to demonstrate meaningful change (statistical table or chart).
