The primary focus of this assignment is to provide students an opportunity to intervene and evaluate interventions with communities. (EPAS 1a/1b, 7, 9)
Students will conduct a Community Needs Assessment to identify an unmet need related to the context of their community-based field agency that can be addressed at the community level. Students will then apply the steps of the problem-solving process to assess the problem and subsequently develop a community-based plan of action to rectify the problem. Finally, students will present a verbal proposal to the class requesting their assistance and support of their proposed solution to the identified community problem or need.
The purpose of this assignment is to assess, plan, and implement a social work intervention in the community. You must include citations from the 2010 Census and at least six (6) recent, scholarly references (from within the past seven years), and a copy of Google Maps referencing the neighborhood you describe. Prepare a reference page in 7th edition APA format. Maximum length is 12 double-spaced, typed pages. Follow these guidelines very closely to help structure your paper; utilize the headers as provided below:
I. Part I: Background and Assessment (60pts)
a. Introduction and Background: Introduce us to the community and the problem you are focusing on. Describe the geographic neighborhood/community, and any historical events (i.e., mills closing, recession) that have impacted it and perhaps led to the problem you are going to address. Narrow this down to one concrete, specific community-based problem.
*The community is Geauga County, OHIO UNITED STATES
*The problem is lack of public transportation. No public buses or trains/ UBER AND LYFT DO NOT SERVICE THE AREA.
* The community is rural. The problem is lack of transportation causing residentsof the community to miss mental health appointments if they can’t find transportation.
b. Analysis of the Population: Include demographics from the 2010 U.S. Census. Describe socioeconomic data, racial and gender mix, industries in the area, etc.

c. Analysis of the problem and its scope: How big or important is it to ‘fix’ the problem? From whose point of view? Who does it affect? What are some consequences of the problem to the community?

d. Hypothesis of Causes (Etiology) of the Problem: What (based on evidence) might have caused the problem in this particular community?

e. Intervention Hypothesis: Supported by scholarly references, suggest an intervention (described in more detail below). What do you hypothesize might be the community members’ response to your intervention? What have others done in similar situations, and with what kind of results?

f. The Problem and Proposed Change/Intervention/Strategy: Choose an appropriate intervention strategy. Discuss the pros and cons of the identified intervention, supporting your choice with the literature. Mental health appointments. Geauga County Job and Family services will
receieve money from the Mental Health Board to transport residents to and fromtheir appointments.
II. Part II: Key Actors and Systems (Total: 120 points)

i. Develop a list of who the stakeholders are who may or would be affected by any proposed change. –

ii. Write Outcome Objectives for the goal.
c. Establish a timeframe: when should the first results be evident? At what point in the future would it be reasonable to expect additional measurable results from your intervention or strategy?

i. Be clear and specific about ways to access funding for your proposal. You do not need to include a budget. Include the letter as an appendix to your paper. [This will become the foundation for your oral proposal.]

i. Describe intended sequence of activities. Which ones need to occur before others start? Who has primary responsibility for ensuring activities are done (monitoring)?
