1- Teaching of the signs and symptoms of diabetes and discussing the impact of carbohydrates, fatty foods, and sugary foods can be done through the use of videos. The teaching plan will involve the entire family and will be implemented after consultation. Hohenleitner et al. (2023) noted that “YouTube can host videos providing information on the pathogenesis, diagnosis, treatment, and prevention of various health conditions” (p. 213). The hospital may provide their recorded YouTube videos to the patients. The outcome of the education is successful.
Presentation of the different types of exercise and its impact in lowering the blood glucose levels can be done using a brochure and PowerPoint presentation. The brochure will show the services provided by 3FIT while the presentation will show the benefits of the different types of exercise on the reduction of calories. Patil and Dhale (2021) explicated that “highly-used patient education medium is pamphlets 90% of the hospitals use this; 80% of the hospitals use digital dynamic PowerPoints through TV” (p. 4). The outcome of the education is effective.
3FIT and LA Care Health Plan are both introduced to the family. The family members were receptive to the suggestions. The family enrolled in 3FIT. They requested L.A. Care Health Plan for nutrition coaching and meal plan.
2-My teaching plan was based on the importance of eating healthy and better food choice habits. I had a discussion with family A regarding how unhealthy food choice habits can affect health. I spoke about the risks associated with eating fast foods and I provided tips on how to lose weight. I also advised the family to eat more fruits. “Most Americans don’t get the recommended five or more servings per day, so look for opportunities to add more fresh, frozen, or canned fruits and vegetables.” (Noble, Dana, 2024)
I implemented this teaching plan by having Family A watch a short video of healthy food habits, I provided them with brochures and allowed them to ask questions. The outcome of the education was beneficial to the family. They were very appreciative of the education provided. Family A was able to verbalize understanding of the importance of eating healthy and exercising. I also shared this with the Family,” Reinforce your new, healthy habits and be patient with yourself. Habits take time to develop. It doesn’t happen overnight.” (CDC, 2023)
I was successfully able to form a collaborative partnership between the community sites and the family. The family was receptive to my suggestions to reach their goals although at first, they wanted to do this alone when they felt that they were ready. I reached out to these resources, “John F. Kennedy Recreation and Aquatic Center and YMCA of Newark and Vicinity,” and connected the family with them. Family A was able to receive useful information that was not so familiar to them and also received referrals to other centers with additional services that were no longer offered.
