Logic Model: Develope a program logic model of your program/ agency that provide a more detailed account of their theory of change. The program should be one that you are currently engaged in at your field agency.

ASSIGNMENT 2:  LOGIC MODEL ASSIGNMENT

Now that you have an understanding of the
agency/program, this next assignment will allow you to identify the theory of change
that drives the agency/program by constructing a logic model, and narrow down
the focus of your evaluation project. Paper should be no more than 10 pages (including logic model)
and a minimum of 7 pages excluding title, logic model, and reference. Be sure
to include an introduction and conclusion. Please ensure you are using a
minimum of 5 peer reviewed articles.

1.      Introduction
(Discuss the social problem, population served, and the problem that the
program addresses) 

2.      Theory
of Change Statement: Brief overview of the strategies
and results of the program. Your theory of change statement should be more
reflective of your program activities and the changes we want to see Ex.“If the
APG program has funding, staff, and a facility, then the program can implement
group counseling, family counseling, and prosocial activities.” You
should then incorporate literature that discusses the outcomes we see from the
activities mentioned. 

 Logic Model: Create a logic model on your program/agency that provide a more
detailed account of their theory of change. The logic model should consist of
the following areas: Please ensure you identify a minimum of 5
Activities,  5 Outputs, and 5 Short-term/Intermediate outcomes.

For every activity there should be an output, short-term
and intermediate outcome that leads to the overall 5 long term outcome.
