Delirium Prevention and Management (Quality Improvement Project Translation Paper)

General instructions: 

Write a 8-10 page paper which describes an evidence-based quality improvement implementation and evaluation process for delirium prevention and management in geriatric patients on a medical surgical floor.  

Describe a clinical problem or issue in need of change. What is the proposed intervention (a small project that can be completed in 3 months), and who will be the target population?

Discuss the literature that address the problem and evidence that support the change.

Describe the theory or framework that would guide the intervention.

Describe the intervention that will address the problem and the implementation steps.

Discuss the evaluation methods and the metrics) to measure success.

Discuss how change can continue or be sustained.

Provide a strong conclusion.

The 10 pages limit exclude the title page, references, and appendices. Please see attached outline, grading criteria, and example papers. APA 7th ed. must be used in this paper.

3. Evidence Supporting Translation (based on the literature and evidence, what intervention(s) have been used to address the problem? List a minimum of two interventions).
