This is Milestone 5, or part 5, of a final powerpoint presentation final project. The overall goal of the project is a plan to expand a hospital’s telehealth services, specifically to use for post-discharge follow-ups and better management for patients with chronic conditions that are at a higher risk of hospital admissions to reduce 30-day hospital readmissions to reduce costs, improve patient outcomes, and improve the populatioln’s health, or to address the Triple Aim of healthcare.
The Week 5 Discussion post and feedback should be used to help answer some of this milestone’s prompts. Some of the questions the professor asks can be answered within these prompts too to enhance the quality of the work. The source used in the post needs to be also used in this milestone assignment.
Preceeding parts of the final project have been provided for reference.
It’s likely that you will need to use/cite some sources from previous parts of the project, particularly regarding the SMART goals for this project which are on the first slide of the Milestone Four presentation. All other citations you use/make from previous milestone assignments must be real and able to be verified. Sources must be credible, preferrably peer-reviewed, and relevant/recent. The source from the change management discussion source needs to be used, and must use at least 1 new additional source which refers to the 1 source required portion of this order.
The prompts on the Milestone Five assignment are the guideline for what to include in the slides, but more importantly, guide the accompanying speaker notes needed to go along with the slides and give more detail. This cannot be bullet points, must be complete sentences, citations when appropriate, and must be written and flow well. All citations in speaker notes And powerpoint slides must be present and in APA 7th edition format.
The slides must also be the same slide layout/background that includes the AuroraHealthCare banner at the bottom, any titles in Montserrat font as in previous slides, and other text in Calibri (body) font as also shown in previous slides/milestones which are included for reference.
Must be at least 5 slides, but can be up to 7 if needed/looks better. No title slide needed. Reference slide must be included but is not included in the minimum 5 slides needed.
Please do not use graphics or pictures that appear distorted or are displeasing to look at.
