Start and Completion Dates: Insert the date you intend to begin your project and the date
you intend to be through with your project. Dates are estimates within the following
parameters:

Work starts no earlier than Thursday, Week 2.
Work ends no later than Friday, Week 6 at 2359 EST.
Work may not be completed during Weeks 1 or 7.

Practicum Project Topic: Provide a comprehensive description of the project with a specific focus
between 50 to 150 words. Your own words should be used. No referenced content is allowed.

Theoretical Framework: Briefly include the following:

Two (2) specific points of the theory stating how each point specifically ties into and/or
supports your project.

Column A: Design 4 specific outcomes to reach the final project goal:
o
Each outcome reflects what is achieved by completing the activities in Column B.
An outcome is not a restatement of the activities from Column B.
o
You may find it easier to start with the activities in Column B then decide what the
outcomes are based on the corresponding activities.

Column B: State the activities you will complete that will allow you to achieve the associated
Outcome:

o
State activities in the format of what you are going to do and number each one.
Do not state what staff, preceptor, patients, or employees, etc. will do.
There is not a required number of activities, but you need enough to reach the 20
hours required onsite and to complete the project.

o Brainstorm the activities you need from the starting concept to the point of completing the
project goal. Distribute the activities within the 4 sections based on how these need to flow
to meet your outcomes and your final goal.

o
Remember that anything you “create” (e.g., quiz, survey, handout, educational tool, policy)
cannot be distributed before your preceptor reviews for you to edit and approves a final
copy for distribution. These activities must be included in Column B.

o
Handwritten content is not acceptable.
A PowerPoint poster template or other electronic format is required.
When creating a brochure, various formats may be used such a PowerPoint, Word,
Canva, etc.

o
Column C: Provide statements that list the content and format to be used to show proof that
you completed Column B’s activities (actual content will be submitted in Assignment 3 due in

Week 6).

o
Review the Sample Activities and Evidence Statements Assistance document & example
evidence statements in the sample LCs in the Week 1 module.

These statements are not a repeat of the activities.
If you start the statement with a verb, you have an activity. Revise.
o
Number each evidence statement to match their corresponding activity.
Last Page Signature and Date: After CAREFULLY reading the documents listed in the
Reading and Resources module so you know what you are agreeing to, sign your name by typing your full
name and then type the date of LC submission.

The date is not when you started the assignment but the day it is submitted for
grading.

Do not change the original date for subsequent revisions of the LC.
Step 3: Submit the LC in the course dropbox no later than Monday, Week 2 by 2359 EST.

o Your faculty will not review a full assignment rough draft prior to submission for grading.

o Only specific questions will be answered.

Early submission is strongly encouraged!

Students are only allowed TWO (2) submissions of the LC to design an acceptable project.

o A 5point deduction will be taken when a 2nd attempt is required in addition to the points
deducted during grading of the first submission.

Step 4: Once your LC has been graded, read the feedback and respond accordingly. The LC will either
be:

1. Approved as presented to be sent to your preceptor OR

2. Revisions are required for resubmission

o Revisions are due within 48 hours of feedback provided; not when you read the email but
from when faculty sent the email.

o Make time in your schedule between Monday and Thursday of Week 2 to check your
email FREQUENTLY to avoid delays in timely submission of the LC to the preceptor!

Step 5: Submit the final version to your preceptor ONLY AFTER receiving faculty approval to do so!
o The final faculty APPROVED LC is due to your preceptor no later than Thursday, Week 2, 2359
EST.

o
Assignment 1 LC grades are posted ONLY when the PRECEPTOR has approved the LC using the
Forms Survey sent out by faculty during Week 1. Therefore, grades may not be posted in the
typical 5 days after the due date.

o
Once the Learning Contract is approved, no changes/edits should be made to any of the
content in Columns A, B, or C in any way (e.g., add, delete, revise, restate, reorder, or rearrange,
etc.).

o
Do not change the date at the end of the LC for Assignment 3.
