For this assignment, you will choose a change idea from your driver diagram (one that you can help improve in your current role), develop a final SMART goal, as well as both a theory of improvement and a PDSA cycle plan.  Please use a conditional (If… Then…) statement to present your change idea.  You may use a template from the NYCDOE Improvement Science Handbook to present your PDSA cycle plan. The PDSA cycle plan must detail the who, what, when, where, and how you plan to tackle your identified problem of practice.
