For this activity, you will create a family care plan, which you will develop to resolve or ameliorate the family needs and issues that were identified through the systemic analysis of the family and the community. You will focus on selecting those resources, practices, interventions, and organizations that are evidence-based and that reflect the least intrusive and restrictive levels of care. Moreover, the care plan should show tight alignment with the family’s identified strengths so they can be leveraged to assist the family to function autonomously as quickly as possible. The knowledge and skills utilized in the development of the care plan will assist students to function as effective practitioners after graduation.

Based on your family and community systems assessment you completed in Modules Two and Three, develop a Family Care Plan using the Family Care Plan Template to continue to address either your family, or the family from the TV show or movie that you recently examined and identify the needed resources, strategies, interventions, and expected outcomes. As you will be required to create Family and Community Care Plans for the Riverez family and the community in which they reside as part of your final project, this task will be used to support you in the successful completion of the final project.
Specifically, the following critical elements must be addressed:
- Problem Statements: Creates accurate problem statements that capture the full essence of the family’s needs and issues, and that are based on the systemic assessment.
- Proposed Interventions: Recommends least restrictive and intrusive interventions that will meet the family’s needs and ameliorate issues.
- Proposed Strategies: Develops appropriate, evidence-based strategies that will efficiently alleviate the needs and issues that were revealed during the systemic assessment.
- Available Resources: Identifies appropriate, evidence-based community resources available to ameliorate the identified needs of the family. Identifies why each resource will assist in meeting the needs of the family.
- Potential Outcomes: Describes potential outcomes from the interventions and resources, including how those interventions and resources alleviate the family needs.
- Overall Assessment: Develops an overall assessment including the systemic family analysis instruments, information derived from the family, strengths, and needs of the family, and impact of the selected assessment.
