Introduction

As a baccalaureate-prepared nurse, you’ll be positioned to maximize the use of technology to achieve positive patient outcomes and improve organizational effectiveness. Providing holistic coordination of patient care across the entire health care continuum and leveraging community resource services can lead both to positive patient outcomes and to organizational improvements.

Preparation

In this assessment, you’ll determine how health care technology, coordination of care, and community resources can be applied to address the health problem you’ve defined. Plan to spend at least 2 direct practicum hours working with the same patient, family, or group. During this time, you may also choose to consult with subject matter and industry experts.

Note: Expect to spend at least 2 hours with the patient, family, or group you’ll be working with For this portion of the practicum, the exploration focuses on how health care technology, care coordination, and community resources can address the identified health problem. Technology plays a critical role in managing the issue, as the patient may use devices such as a blood pressure cuff, pulse oximeter, or glucose monitor for regular monitoring. Smartphone apps could also assist with managing the condition, and telehealth may be a viable option for providing ongoing support and consultations. Additionally, the patient may rely on websites to obtain more information and resources related to their condition. Regarding care coordination, services such as home care, physical therapy, or dialysis could be instrumental in managing the problem effectively. Transportation availability is another critical factor, ensuring that the patient can attend necessary appointments. Community resources, such as support groups and religious institutions, might also play a vital role in addressing emotional and practical challenges related to the health issue. These combined approaches provide a comprehensive plan for addressing the health problem and enhancing patient outcomes.
