Assessment, Findings (Recognizing Cues) – Potential assessment findings in this patient population that could warrant further investigation Ex- vital signs subjective & objective, labs, diagnostic tests, and risk factors, including psychosocial components that may affect the patient and family. 3. Problem Statements (Analyzing Cues): What is this patient’s highest priority issue? Is it an actual problem or a potential risk? Or both? Please include at least Three.a. Ex. Impaired gas exchange, electrolyte imbalance, death anxiety, risk for infection, malnutrition risk, and increased risk for venous thromboembolism risk (VTE). 4. Planning and Goal Setting(Generating Solutions)– Using critical thinking to support your clinical judgment, what would be good outcomes for your patient? For the Three problem statements discussed above, discuss SMART goals for each problem statement individually.
5. Interventions (Taking Action): what will you do to help improve your client’s condition or prevent further deterioration? Ex: Diagnostic Procedures and Nursing Interventions, Labs Collaborative Care- Interprofessional approach Medications, therapies, and treatment for critical care patients. Vigilance is necessary to prevent the worsening of the condition and possible death. 6. Evaluating Outcomes: How can nursing actions result in the desired outcome for this client?
