Evidence-Based Practice Project Proposal: Identification of Nursing Practice Problem

This assignment is for an ACNP program and will be in different parts, the purpose is to help form a PICOT question. I will attach the questions needing to be answered at the bottom. The subject i would like to do it on is sepsis screening identification impact on sepsis alert patients decreasing morbidity/ mortality/ hospital rates.  PLease fill out or complete the form questions on the bottom part where it starts by saying PICOT draft.

The purpose of this assignment is to select a relevant nursing practice problem for your evidence-based practice project proposal. To identify a relevant problem, consider problems generally faced in nursing practice (coordination of health care, assessment, education, patient support, trauma prevention, recovery, health screenings, etc.). Use the “PICOT Draft” template to complete this assignment.

Use a national, state, or local population health care database to research indicators of disparity. Choose a mortality/morbidity indicator to identify a clinical problem or issue that you want to explore pertaining to a population of focus. Use this indicator to begin to formulate a PICOT (Population/Problem, Intervention, Comparison, Outcome, and Time to achieve the outcome) statement.

To find a national, state, or local population healthcare database for researching indicators of health disparity, you can explore several reputable sources:

1. **Centers for Disease Control and Prevention (CDC):** The CDC offers a wide range of databases and tools that provide data on various health indicators, including disparities. The CDC WONDER database is particularly useful for detailed health-related data.

2. **United States Census Bureau:** This bureau provides extensive data that can be helpful in understanding demographic aspects of health disparities.

3. **Agency for Healthcare Research and Quality (AHRQ):** AHRQ’s Healthcare Cost and Utilization Project (HCUP) and Medical Expenditure Panel Survey (MEPS) are valuable resources for health services research.

4. **National Center for Health Statistics (NCHS):** Part of the CDC, the NCHS offers data sets that are crucial for public health research, including disparities.

5. **State and Local Health Departments:** Many states and localities have their own health data systems. These can be more specific and relevant for community-level health disparity research.

6. **The Kaiser Family Foundation (KFF):** KFF provides state-level data on a variety of health issues, including disparities.

7. **Behavioral Risk Factor Surveillance System (BRFSS):** This system, managed by the CDC, is a collection of health-related data from state and local health departments.

8. **National Institutes of Health (NIH) Databases:** NIH offers several databases through its various institutes, such as the National Institute on Minority Health and Health Disparities.

9. **The Dartmouth Atlas of Health Care:** Provides data and analyses about national, regional, and local markets, as well as hospitals and their affiliated physicians.

10. **Robert Wood Johnson Foundation’s County Health Rankings & Roadmaps:** Offers data on various health indicators at the county level.

Problem Statement:

Step 1: Identify Your Population of Focus and Health Issue
First, you need to identify the population you are interested in (e.g., elderly, children, a specific ethnic group, patients with a certain condition) and the health issue or clinical problem you want to explore (e.g., diabetes, heart disease, mental health disorders).

Step 2: Choose a Relevant Mortality/Morbidity Indicator
Based on your population and health issue, choose an indicator that reflects the health outcomes or the severity of the problem within this group. This could be:
– Incidence or prevalence rates of a disease
– Hospitalization rates
– Mortality rates associated with the condition
– Rates of complications or comorbidities

# Step 3: Formulate Your PICOT Question
With your population and indicator in mind, start formulating your PICOT question.
