Magnitude of the problem – present current and relevant statistics at the national level
Population(s) of focus/priority population – present and interpret statistics that provide support for a population(s) of focus
Consequences if the problem is left unchecked
Costs (in terms of dollars) of the problem

State a goal and 2 objectives for your specific population (because some broad goals might not be culturally relevant or specific enough)?
What would be the baseline and outcome (short-term) evaluation tools to determine change over time?
Describe an ideal data collection strategy (e.g., sampling, timing)
What are the strengths of the tool?
What are the limitations of the tool (at least 3)?
Would there be any adaptations to the tool & why?
How will changes be tracked/evaluated over time?
How will the baseline assessment be measured? (specific to your community – how would you do this)

Supporting components will include a description/justification of community partners, a brief policy analysis, recommended interventions, and assessment of community impact (implications). (2 pages):
Community Partners: Identify 2 existing community partners (in the community you’ve identified) and provide a description of the organization and why you would partner with them.
Policy analysis: Identify 1 local/regional/state/federal policy related to the focus of your assessment. Describe a policy change that would most increase the agency’s effectiveness and provide a rationale.
Recommended interventions: Use research to inform practice, and recommend 3 interventions to address the challenges in your population. Intervention has to be on the population level and have demonstrated effectiveness. You should provide effectiveness data in your write-up.
