Students will practice professional documentation by completing six (6) case notes throughout the semester. Each meeting with their “client” will correspond to different theories or models covered in class. In addition, students will create a journal entry to reflect on their practice experience with their “client” and with the different modalities.
The case notes will serve as progress monitoring for the self- care project and will follow the guidelines below:
- Students will use page 1 of the SOAP note template provided on Canvas to complete their case notes.
- Journal entries are expected to be reflective, thoughtful, and written with complete sentences, correct spelling, and proper grammar and punctuation.
- These case notes and journal entries are expected to be 2 pages long each, one page for the SOAP note and one page for the journal entry.
