For this option, you will pick one of two case studies. You will analyze the chosen case study and create a report. Note: Each case study has a different purpose and therefore different requirements for completion.
Case Study #1: Cover Shield Failure and Falling Incident at a Hydroelectric Generating Station

Requirements for Case Study #1
- Develop a high-level briefing note directed at senior leadership.
- Create a PowerPoint presentation to senior leadership as though you presenting an overview of the incident and making recommendations.
- Provide next steps and direction for senior leadership’s consideration and approval.
Using the 5 why’s root cause analysis approach, create a report that will highlight a number of causes that contributed to the failure of the cover shield and falling incident at the
hydroelectric generation station.
