Instructions:
Rationale Reflection upon clinical practice is a critical part of being a health practitioner. Reflection allows the practitioner to learn from their experience and grow. It also helps practitioners contextualise their experiences.
Task:
During placement, students will attend a range of cases. Each case is a unique learning opportunity. Students should choose a case that caused them to reflect on practice. This may include reflection on their own performance, reflection on their biases or assumptions, reflection on their education or personal reflection. Students should report on this case ensuring they address the following elements:
1. What is the environmental, social and cultural background of the case? What factors influenced this patient’s presentation. Could any of these factors be altered through addressing the patient’s health literacy, care plans or social determinants of health?
2. Succinctly describe what is the clinical nature of the case? What is the basic physiology behind the presentation? Has the presentation been influenced by any clinical factors such as chronic disease? 3. How was the case managed clinically? Briefly outline interventions and the decision making underpinning the use, or omission of various clinical options.
4. Was the care of the patient tailored to ensure it was patient centred? Refer to communication, referral options and other supports provided to the patient.
