  2. Identify the main health problem, concern, or risk. Make sure to provide evidence based on your summary of the community assessment.
  3. Identify the population this community problem influences the most and explain why.
  4. Explore if this problem is identified in Healthy People 2030. Discuss the Healthy People 2030 vision to improve this problem.
