An understanding of cells and cell behavior is a critically important component of disease diagnosis and treatment. But some diseases can be complex in nature, with a variety of factors and circumstances impacting their emergence and severity.
Effective disease analysis often requires an understanding that goes beyond isolated cell behavior. Genes, the environments in which cell processes operate, the impact of patient characteristics, and racial and ethnic variables all can have an important impact.
An understanding of the signals and symptoms of alterations in cellular processes is a critical step in the diagnosis and treatment of many diseases. For APRNs, this understanding can also help educate patients and guide them through their treatment plans.
In this Assignment, you examine a case study and analyze the symptoms presented. In 1-2 pages, you will answer the questions provided following the case scenario. You must use current evidence-based resources to support your answers. Follow APA guidelines. Follow the grading rubric.

Medical errors have a notable impact on healthcare, most especially with young people who have mental disorders and impairments. These errors can occur in different
phases of patient care, like medication management, therapeutic intervention, and diagnosis, which can result in adverse effects, such as rashes, hives, dizziness, and headaches. These might increase hospitalization rates and reduce accountability among healthcare providers, Chen K., et al., (2022).
Several databases provide information on medication errors, especially in health facilities, pharmacies, and hospitals. Notable ones are electronic Medication Administration Records (eMAR), Pharmacy Databases, and Adverse Event Reporting Systems like FDA’s MedWatch and Insurance and Hospital Incident Reporting Systems. For instance, eMAR data is accessed by authorized healthcare professionals with authorized login codes, such as Nurses, caregivers, and health providers, to monitor patients’ charts and administer medications. FDA Medwatch provides medical product safety reporting for health professionals, patients, and consumers, as well as prescription and over-the-counter medicines. US FDA (n.d). However, the application of databases has limited use in mental health management due to the complexity and sensitivity of information required in mental health disorder management, Kariotis et al. (2022).
Impact of Medication Error Database on Mental health Disorder
Worldwide, 5% to 41.3% of all hospital admissions are due to medication errors, and it is reported that 30% is higher in patients who are receiving five or more prescribed medications, Tariq., et al. (2024). The information database aims to prevent misdiagnosis leading to inadequate treatment. Although electronic medical history records are widely used in healthcare delivery,
Medication errors may result in improper pharmacological interventions that increase symptoms Rasool et al., (2020). Furthermore, errors can occur in psychotropic medication prescriptions due to incorrect dosing or drug-to-drug interactions, which can lead to serious drug side effects, such as dizziness., insomnia, or increased hallucination, Robin, L., (2024). Additionally, due to communication gaps, youths with mental disorders commonly experience disorganized care among providers, which may result in conflict in treatment care plans and medication reconciliation issues, Merikangas et al. (2013). These medical errors can bring about increased hospitalization rates and high healthcare costs, which can cause a bearer in the mental and emotional well-being of young people with mental health disorders.
The application of Information Technology (IT), such as automated dispensing, barcode medication administration, and electronic medication reconciliation, can reduce medication errors in young adults with mental health disorders by encouraging patient and caregiver involvement in the treatment care plan, implementation of a clinical decision support system (CDSS) Agrawal (2009). Also, IT applications enhance provider education and training and Improve communication and collaborative care among interdisciplinary teamwork such as psychiatrists, nurse practitioners, and psychologists to deliver adequate treatment management Agrawal (2009).
Conclusion
Medical errors in the youth population with mental disorders create a substantial risk to their well-being and long-term mental health stability. By practicing
evidence-based strategies, for instance, enhancing education, clinical decision support systems, and encouraging communication with patients by involving patients
in treatment plans and continuous monitoring for safety; with these steps, healthcare professionals can significantly reduce the causes of medication errors in mental health disorders and enhance patient care.”
