Final
Report
As part of the Analyze and Improve phases of the DMAIC
model, you learned the importance of correctly identifying a root cause before
formulating improvement solutions. The final step is to implement Control phase
tools to ensure that improvements are sustained at the Shelbyville Medical
Center. The direction and communication required to successfully implement the
Control phase is contained in the process control plan.
The final
report is the third task of the assessment. To complete the report, you must
demonstrate an understanding of:
- the importance of the Control phase in maintaining
quality improvement - the tools necessary to assess the impact of a quality
improvement initiative on an organization - the components of a successful control plan
Follow these
three steps to help you complete the final report:
Discover: Review the importance of the Control phase and assess
how quality improvement initiatives can be sustained at an organization.
Engage: Explore the components of a successful process control
plan and consider how they can be implemented at Shelbyville Medical Center.
Review: Apply what you have learned to complete Task 3 of the
Quality Improvement in Healthcare assessment. For more detailed requirements,
visit the Assessment area on the Course of Study landing page.
Essential Materials
Use the following resources to complete the final report.

Performance Improvement: A Lean Six Sigma Approach prepares healthcare administrators to
meet the challenges of a changing marketplace through the Lean Six Sigma method
of quality improvement.
Miller’s Impact
Assessment Framework provides an
in-depth guide to assessing the potential impact of quality improvement on the
multiple dimensions of the organization.
Task 3: Final Report Template
Six
months after implementing the Improve phase, members of the CQI team at
Shelbyville Medical Center performed audits to determine the success of the
initiative. They found significant improvements in several areas:
- Patient
satisfaction scores achieved or exceeded national benchmarks. - Patient wait
times had decreased significantly. - The daily volume
of urgent care patients continued to increase.
To ensure that the improvements are sustained,
the Control phase needs to be implemented. In the final report, you will create
a control plan. In this control plan, you will ensure that quality improvements
remain consistent by:
- determining what
measures should be used to track progress - developing a
response plan if measures fail to meet the standards of the project
charter - developing
control methods to ensure that performance remains consistent - identifying who
in the organization should receive the relevant data on quality
improvement - determining what
remedial actions will need to be implemented to ensure that these
improvements can continue

Now
that you have reviewed and explored the tools involved in the Control phase of
DMAIC, you can begin to apply those tools to quality improvement at Shelbyville
Medical Center. After the Improve phase of DMAIC, metrics show that the quality
improvement initiative has been a success.
What Are the Elements of a Successful Process Control
Plan?
Tools
and tactics that will be used to monitor and sustain the initiative
improvements should be outlined in a process control plan. To prepare to
develop the process control plan and final report for the quality improvement
initiative at Shelbyville Medical Center, explore the basic elements of the
following process control plan.

Final Report
Complete
the Control phase of DMAIC by preparing a final report on the quality
improvement initiative that outlines the progress made as well as plans to
monitor and control improvement.
For this part of the assessment, you will
