While vaccine hesitancy has been documented for much of human history, the rise in social networking has proliferated vaccine refusal/hesitancy.

1) What is the history of vaccine hesitancy?  Is it increasing, why? 
2) What risk does vaccine hesitancy/refusal play in disease outbreak?
3) Should public schools and health care facilities/procedures require vaccination?  Who should be held responsible if school or daycare children get seriously ill or die from vaccine preventable diseases?
