Part 3 – Dietary Intake, Physical Activity Level & Mood Analysis Final Evaluation Narrative (4
required steps. See below.)
Students will write final
evaluation paper, at least 3-4 full
pages long (normal margin (1”) Times New Roman12-point font).
The Final Evaluation/Write-up discuss your food intake in
relation to its nutrient content. It must include the following:
Ø A
comparison of your baseline assessment and your post assessment results.
Meaning you will discuss the difference of your actual food intake and
exercise/physical activity (before and after) and the dietary guideline
recommendations of www.chooseMyPlate.gov.
Ø
Discuss if you were able to follow the plan that
you wrote in your baseline assessment write up.
Ø Write
what improvements, if any, did you noticed between your baseline assessment and
the post-assessment (compare the assessments)?
How much improvement was made?
Name and describe the exact improvements.
Ø What
did you learn about yourself and your habits?
Ø What
did you learn about your mood? Identify and describe how you felt after eating
a certain type of food or drinking a certain type of beverage?
Ø How
easy or difficult was it for you to stick to your plan?
Ø At
what “Stage of Behavior Change” are you now in? Explain why you are in that
stage. Remember to draw the cycle.
Ø Discuss
your height and weight, calculate BMI, and explain what it means in
terms of your personal health, weight history, and age. Show your work.
Ø Discuss
what foods/beverages you eat in
excess but are not recommended in the ChooseMyPlate website and what
are the consequences of overeating those foods/beverages. Discuss it in tern of
the nutritional value of the food item and the medical risk associated with it.
Example: “I drank too much soda. Soda provides too much sugar and empty
calories. If I continue with this habit, it can result in obesity.”
Ø Discuss
what food/beverages you do not
eat but is recommended in ChooseMyPlate website. Why do you tend not to consume
those foods/beverages? Explain why portion control is important. What are the
consequences of not eating those foods/beverages? Note: This should be discussed
in terms of the nutritional value of each food item and the medical risk
associated with each food item.) Example: “I do not drink enough milk or dairy
products. I am not consuming foods that have enough calcium and vitamin D, which
could result in low bone density and heart disease when I am older.”

your intake of meat/poultry- protein, the recommendation and its health
benefits

your intake of milk and its products, the recommendation and its health
benefits
Ø Discuss
your intake of sugar, the recommendation and its health benefits
Ø Discus
your intake of water, the recommendation and its health benefits
Ø Discus
your intake of fiber, the recommendation and its health benefits
Ø Discuss
your intake of whole fruits, juices or fruit drinks; what’s the
difference between and what’s their benefit or problem in consuming them
Ø Discuss
your intake of vegetables– dark green and orange versus starchy
vegetables and legumes and what’s the health benefits
Ø Discuss
your intake of oils and fried foods, the type of oil/fat to eat, the
recommendation for saturated oil, Trans-fat, Omega 3…
Ø Discuss
your intake of sweets and junk foods and the effect on your weight
Most Important
– Discuss the connection between your diet, physical activity level, dietary
recommendations, possible nutrient excess or deficiency, health status and your
actual weight.
In the final evaluation/write up paper, you must discuss how
you have improved (or not improved) your health, dietary behavior and physical
activity level. Also, provide a discussion of the barriers you have overcome
and/or are still facing?
You should be able to discuss in detail what factors have
contributed to your poor health and eating behavior.
Note: For this
assignment you must only submit the Final Evaluation paper. You don’t need any
other forms from the Baseline or Post-Assessment assignments.

Complete all components of the assignment and
follow ALL instructions.

Be honest. You are not being judges. It is fine
if you are not able to change your eating habits over the course of a few
weeks. The purpose of this project is to help you understand yourself better.

Turn your journal in on time.

ensure that your work is complete.
