Integration of Public Health and Primary Care Concept Diagram/ Concept Map/Asset Map

There are two files I uploaded that have examples on how to do it and this instruction text i posted on here: For this
assignment, you will chose a public health issue to focus on and propose ideas
for addressing the issue through integration of public health and primary care.
This is an opportunity to be creative and synthesize the information you’ve
learned in this course, as well as others. The idea is to create a concept
diagram or model of determinants of the health issue and strategies for
addressing the issue. Your model should be presented in a visual format or
diagram and include the following 1) social determinants of the health issue;
2) consideration of policy, systems, and environment; 3) role of public health
in addressing the issue; 4) role of primary care in addressing the issue; 5)
partnerships that should be leveraged to address the issue; and 6) potential
areas of collaboration between public health and primary care.
