a.Your task is to research at least 3 agencies, 

–one (1) local school district or college, 

–one (1) state or federal department, and 

–one (1) community-based organization.  ( location, NY)

Each must be involved in offering transition supports to students/people with disabilities.

b.You will be looking for the following: 

 Is there a specific person or office that coordinates services and support? What types of transitional services and supports are offered? 

1. 1. 1. Does the organization offer assessments for people to determine their skills, talents, preferences, and future goals>
      2. Are there any reviews online regarding the effectiveness of the services offered by these organizations?
      3. After reviewing the organization, analyze your findings.  Would you recommend each organization?  Do you feel each group adds value in the transition process, or do you think some of these groups are less effective than others? 
      4. REMEMBER to include references and information that demonstrates where you found your information, including any conversations you might have had.
      5. 1. Include a page at the end listing all the references you used or assembled. 
         2. You are required to create grids or graphs to show your findings and comparisons.
