The IPA this week has two parts for you to answer.
- Part One: Work with your field instructor to develop and in this IPA write a summary of your end of the semester transition plan.
- Part Two: As you look back over the semester, share two examples of client and/or staff transition or termination planning meetings. Include one example that went well and what strengths or skills you demonstrated that made it so successful. Share one that did not go so well and what you learned from it.
Notes for writer:
1. I met with my field instructor and there is no transition plan needed because I don’t have my own caseload. When I return from break I will resume my duties working with the LIMA court. The plan for when I return is to manage one or two clients on my own. I will start by assessing the client and completing the intake process. I will then assist the client in finding a treatment program to go to them stable housing. I will also assist them in obtaining vital records if needed and resources to become connected with the recovery community.
2. I have the opportunity to assist with several discharge sessions with clients in the Law Enforcement Intervention for Mental Health and Addiction (LIMA) program. These meetings are very positive. Each client completed the one year program successfully. They were able to remain sober, finding stable housing and they became connected to the recovery community. This past week I helped facilitate a group with 20 graduates. We met with them to complete discharge paperwork and prepare them for a big graduation ceremony that the Metro police department and Clark County Court is having for them. The graduation will have many elected officials attending and speaking to the graduates. It will also be televised.
The only example I have of a bad termination is when a client was terminated from the Felony DUI program due to non-compliance. He was caught drinking at a bar where he then ended up committing another crime. When they remanded him in court he was yelling and screaming at the judge. He was sentenced to 3 years in prison.
