Operating and Support Hazard Analysis including Indentured Equipment List (IEL), Indentured Task List (ITL), O&SHA Worksheet and up to 10 – Page Report

This is a 2 part assignment for M2 Bradley fighting vehicle maintenance ground hop O&SHA.  Follow the Chapter guidelines on building O&SHA from the attatached book. 

The first part is the attached Excel spreadsheet creating O&SHA with the task list applicable to the ground hop maintenance operation. Create this O&SH according to guidance in the Ericson Hazard Analysis Technique book. Make sure to apply the 5 principals of Human performance from conklin to the causes and recommended actions in a similar way to the Attached example PHL document. The task list should have all the steps listed but not all steps presents an hazard. 

  • O&SHA: At least 20 Entries
    • The 20 entries may be from a single Indentured Task List for a single operation or could be a combination of operations and Indentured Task Lists. Some tasks have fewer steps than others 
    • Task entries with no hazards will not count toward the minimum.
    • Your cause explanations should include at least one element related to the 5 Principles of Human Performance. You may include multiple causes for a single hazard entry, but at least one of them needs to relate to the Principles of HP. 
    • Your Recommended Actions should include at least one Recommended Action that somehow addresses the Principle(s) of HP listed in your cause statement.
Second part of the assignment is the 7 page report. Follow the guidelines below also you can take a look at the attached PHL report. You can use the attached Technical manuals as resource on the M2 Bradley. Follow the guidelines below for the structure of the report.

Report Guidelines:

            –ii. Supporting graphics/photos or sketches of significant equipment/system schematic diagram.

E. Discussion of Methodology of Analysis for the O&SHA

   –i. Description of how you adapted Ericson’s step-by-step process table

  • Follow the O&SHA process table in the O&SHA chapter of Ericson’s textbook

    –ii. Reference or include appropriate checklists (for example, from Appendix C in Ericson’s textbook)

    –iii. State any assumptions used for the analysis

    –iv. Indentured Equipment List, Indentured Task List(s) , TLM list and SCF list.

    –v. Include description of risk assessment matrices, IMRI/FMRI, explain your definition of acceptable risk in terms of probability and severity from your risk matrix, and describe how you used the ANSI Z10 Hierarchy of Controls and 5 Principles of Human Performance to reduce risk to acceptable levels. 

     -vi. Explain how you applied the 5 Principles of Human Performance to the O&SHA process and how you used the 5 Principles to develop Recommended Actions/Risk Controls to address hazard Causes (thereby reducing probability of occurrence) and/or to minimize the Effect Severity.
