THE APPLICATION OF HUMAN FACTORS ANALYSIS AND CLASSIFICATION SYSTEM (HFACS) TO INVESTIGATE HUMAN ERRORS IN AVIATION ACCIDENTS.

The bachelor’s thesis deals with the issue of air accidents caused human factor.

3. OBJECTIVES OF THE CHOSEN TITLE

The goal of this bachelor’s thesis is to create an analysis of aircraft accidents that were caused
by human error in civil transport aviation, with a focus on how the human factor played a role.
Moreover, the goal of this study was to look at aircrew, environmental, supervisory, and
organisational factors linked to commercial aviation accidents by using the Human Factors
Analysis and Classification System (HFACS).

4. REASONS FOR SELECTING THE TOPIC

To determine the causes of air accidents and serious incidents, and how safety

recommendations intended to prevent recurrence.

5. EXPECTEDOUTCOMEOFTHESTUDY

By using the HFACS model, safety investigators may get a better understanding of the factors
that lead to accidents, allowing them to plan and implement safety measures to avoid these
problems.

6. PRELIMINARY LITERATURE REVIEW

The Human Factors Analysis and Classification System (HFACS) is a method for identifying and
analyzing factors that contribute to aviation accidents. Previous research has shown that
HFACS can be used to find general trends in the human factors that causes military and general
aviation accidents. The HFACS framework support the investigation process and focus on
prevention measures and training programmes. Investigators will be able to carefully identify
organisational flaws that led to an accident as HFACS addresses human error at all levels of the
system, including the condition of aircrew and organizational factors. HFACS covers four levels
of human error in the aviation system (1) unsafe acts of operators, (2) preconditions for unsafe
acts, (3) unsafe supervision, and (4) organizational.

7. RESEARCH METHODOLOGY

The theoretical section of my thesis discusses the reasons of human failure and explains
HFACS method that seeks to identify and evaluate this failure. The second part I will be
analysing statistically commercial aviation accident data from years 2013-2018 obtained from
databases maintained by the National Transportation Safety Board (NTSB) and Aviation Safety
Network (ANS).
