Context
For this assignment, you will be using a Theory of Change model to help further plan your intervention. A Theory of Change model provides a road map of the inputs, activities, outcomes, and goals of an intervention.
Assignment Instructions
In this assignment, you will outline the internal and external stakeholders, the activities each stakeholder will partake in, and the short-term and long term goals of the intervention. Use the Theory of Change Template [PPTX] Download Theory of Change Template [PPTX]to complete your submission for this assignment.
- Note: Please delete the explanatory text from the template before turning in your assignment. The explanatory text is there to help guide you through the process of creating your own Theory of Change model, but should not be included in what you turn in.
A clear Theory of Change (ToC) model connects inputs to activities to outputs in a way that someone viewing the model can easily interpret. Another way to think of this is, you want to ensure that any of your stakeholders could pick up your ToC and understand their role and how they would impact the any inputs, activities, and outcomes of the intervention. As you are working on your assignment, be sure your ToC is comprehensive and includes:
- Resources/inputs (organizational plan).
- Activities (stakeholder/participants actions).
- Goal.
- Measures of success.
- Outputs.
- Outcomes.
- Impact.
