- The therapeutic goal of carbidopa-levodopa in the treatment of Parkinson’s disease is to balance dopamine and acetylcholine in the brain’s basal ganglia by enhancing dopamine delivery to brain cells. The motor symptoms of Parkinson’s disease are subtle movement tremors that can gradually get worse which can lead to widespread movement disorders. “The symptoms associated with parkinsonism are caused by a deterioration of the dopaminergic neurons in the substantia nigra pars compacta, which results in a depletion of dopamine along the nigrostriatal pathway that extends into neurons in the autonomic ganglia, the basal ganglia, and the spinal cord and causes progressive neurologic deficits.” (Willihnganz, 2023) Carbidopa is highly effective when combined together with levodopa. By combining carbidopa with levodopa, carbidopa prevents the premature conversion of levodopa to dopamine in the peripheral tissues. This approach allows a greater amount of levodopa to reach the brain before being converted to dopamine, increasing the drug’s effectiveness in return reducing the required dose of levodopa. Carbidopa acts as an enzyme inhibitor which also reduces the metabolism of levodopa in order to allow more levodopa to reach its target in the brain. Parkinson’s disease is a degenerative and incurable disease with the goal of treatments is to ease symptoms and reduce disease progression. Medications prescribed for Parkinson’s disease mainly affect the central nervous system.
